Tobacco Use Form
Form to be completed by employer to identify employees who frequently use tobacco. Per ACA, beginning 2014, carriers can rate based on tobacco use. Employers will receive form 7 months prior to anniversary, and must return with 2 weeks of receipt.

2013 MLR Group Size Attestation Form

Employer ID Number - Request Form
CMS requires that IBC provide CMS with the EIN for any Medicare Advantage or PDP customer active during 2012. In order to comply, IBC has mailed this sample letter and form to all affected clients, asking for return by 2/8/13.

Billing & Collection Agreement (300+ size)
This agreement will be required for groups with 300+ enrolled lives. As of 1/1/13 renewals, IBC will no longer include commission as part of their premium. Rather they will collect a service fee for the broker, for which this agreement is required.
