Billing & Collection Agreement (300+ size)
This agreement will be required for groups with 300+ enrolled lives. As of 1/1/13 renewals, IBC will no longer include commission as part of their premium. Rather they will collect a service fee for the broker, for which this agreement is required.

Audit Package
IBC will randomly audit their clients to assure that they meet their UW requirements. Your clients may be sent this audit package, which must be completed timely and include tax documents. Failure to comply, can result in policy termination.

Plan Sponsor Certification form
IBC branded form that will be sent to employers who have not made a benefit change. Employer must complete and return to IBC in order for IBC to know if they do or do not have a GF'd plan. This will affect information then mailed to members.
